Abstract

Summary: Two cases are reported to demonstrate the coincidence of Parkinson's disease with other diseases of the central nervous system. In case 1 a chronic multiple sclerosis was associated with a lesion of the substantia nigra as found in Parkinson's disease. The prominent clinical features during the last years of life were progressive rigidity and diminution of voluntary movements. The coincidence of multiple sclerosis and Parkinson's disease is uncommon and not hitherto reported in the literature. The second case had long suffered from an extrapyramidal syndrome typical of Parkinson's disease. During the last years before her death she developed mental disease with hallucinations and dementia. The neuropathological examination revealed Pick's disease of the type involving the temporal lobe, combined with a severe diffuse lesion of the substantia nigra. In connection with these pathological findings the role of the damage of the substantia nigra as second order site of manifestations in degenerative diseases of the nervous system is discussed. A review of the pertinent literature is included.

<p>Zusammenfassung: Es wird über zwei Fälle berichtet, bei denen ein Parkinson-syndrom mit anderen zentralnervösen Krankheitsprozessen kombiniert war. Bei Fall 1 handelte es sich um eine chronische Multiple Sklerose, bei welcher sich klinisch eine zunehmende Versteifung und Antriebslosigkeit entwickelte. Die neuropathologische Untersuchung ergab eine chronische MS sowie eine Nigraschädigung im Sinne einer Paralysis agitans. Das Zusammentreffen dieser beiden Krankheitsbilder wird als zufällig bezeichnet bzw. auf die Einmaligkeit dieser Kombination verwiesen. Der zweite Fall war klinisch durch lange Zeit als extrapyramidales Syndrom, vor allem als Parkinsonsyndrom aufgefaßt worden. Erst später entwickelte sich eine psychiatrische Symptomatik mit Halluzinationen und Demenzzeichen. Die neuropathologische Untersuchung ergab eine Picksche Erkrankung vom Temporaltyp und eine schwere diffuse Nigraschädigung, die als echte Nebenlokalisation der Pickschen Erkrankung aufgefaßt wird. Im Rahmen dieses Falles wird unter Berücksichtigung der einschlägigen Literatur die Frage der Nebenlokalisation im allgemeinen und bei der Pickschen Erkrankung im besonderen hinsichtlich ihres pathogenetischen Stellenwertes diskutiert.</p>

Bei Fall 1 handelte es sich um eine chronische Multiple Sklerose, bei welcher sich klinisch eine zunehmende Versteifung und Antriebslosigkeit entwickelte. Die neuropathologische Untersuchung ergab eine chronische MS sowie eine Nigraschädigung im Sinne einer Paralysis agitans. Das Zusammentreffen dieser beiden Krankheitsbilder wird als zufällig bezeichnet bzw. auf die Einmaligkeit dieser Kombination verwiesen. Der zweite Fall war klinisch durch lange Zeit als extrapyramidales Syndrom, vor allem als Parkinsonsyndrom aufgefaßt worden. Erst später entwickelte sich eine psychiatrische Symptomatik mit Halluzinationen und Demenzzeichen. Die neuropathologische Untersuchung ergab eine Picksche Erkrankung vom Temporaltyp und eine schwere diffuse Nigraschädigung, die als echte Nebenlokalisation der Pickschen Erkrankung aufgefaßt wird. Im Rahmen dieses Falles wird unter Berücksichtigung der einschlägigen Literatur die Frage der Nebenlokalisation im allgemeinen und bei der Pickschen Erkrankung im besonderen hinsichtlich ihres pathogenetischen Stellenwertes diskutiert.</abstract>
